Please fill out this request in triplicate, including all pertinent information regarding the proposed improvement. Submit your request (all three copies) to the

Architectural Committee. An approved copy will be returned to you, one copy will be retained by the Architectural Committee, and one copy will be filed with the Board of Directors. This approval does not relieve applicant from obtaining the necessary building permits from governmental agencies involved.
